It kind of looks like that switch wasn't rated for the amount of power it was switching and that it should have been disconnected further upstream before flipping that switch.
But I'm not trained in power electronics so I could be wrong.

Any time you turn on a large feeder like that you're always on edge. Even if you're 100% sure you did everything right.
I do that even when it is a small braker in my house.

And this is why we spend so much time training people to do this properly. This is why proceedures have checks, lockouts, extra redundancy, and are generally engineered to be as blunt and simple as possible when it comes to high current situations.
